NCCN has recently completed the development of an online tool to support the Shared Care Agreements process. The tool will facilitate communications both within Shared Care Centres and between Shared Care and Specialist Centres and create an evolving working record of service delivery arrangements.NCCN will be introducing the tool to Centres during site visits over the next 12 months. Members of Shared Care teams will be able to access the tool through a link on this website using their standard NCCN portal login.
